We beat this topic to death where I work in safety management for special trades companies......fraud is common and lack of supervisory skills in this area is a considerable factor in this area too. A claim gone bad because the incident was not reported but the injury was used for a hospital ER/treatment shopping spree can wreak havoc on any safety record and program effectiveness.

Supervisors understanding the need to "supervise" and given the tools and backing to do so can significantly help control these costs and the development of bad behaviors. Talk is cheap and ineffective, statements of policy backed by support translated into action and/or discipline work.

Also, if you are using reporting forms of some sort - make sure you incorporate a "supplemental form" as a tool as well. This form will allow for other reporters, follow up info, statements and outcomes to be easily captured during or after your investigation. Support the use of this process tool by asking management or supervisors how they would identify and locate persons at the scene years after the fact for court or additional statements .....after they no longer work/live or had been identified properly.
