i use a disclaimer on all of my safety review reports to clients (we are a broker) and it basically states that we suggest and that our suggestions are only a guide. Furthermore it states that any suggestions or comment listed reflects observations noted only on the date the visit was conducted. Moreover it says, the findings and suggestions are advisory and we assume no responsibility for the control or correction of conditions noted. I know this wouldn't help our client in a fatality but it sure would protect us from being dragged into a lawsuit.
